Step Two:
Submit your documents
Now that you have completed the new client registration packet, the first phase of the intake process begins! Please fax or mail the following items (attn: Center Director) relative to your case for review:
a. Copies of any protective orders filed
b. Current court order
c. Any Judicial Council form relating to supervised visitation orders
d. A report of any written records of allegations of domestic violence or abuse
e. An account of the child(ren)'s health needs if the child(ren) has a chronic health condition
f. Completed client registration packet
g. The party ordered to pay must also submit two (2) recent check stubs or verifation of current financial income
Other public information may also be obtained. These documents will be reviewed to assess the nature and degree of risk for your case.
